Playhouse
Playtime Playhouse a "press out and set up" book which was published in 1968 by Western Publishing Company, and is really amazing. Don't take my word for it though, you can see all the pages here. It is the kind of book I might scan, print out and assemble, but I could never really punch out the pages from the book!

The front has the name Jason Lee signed under the drawing, but there are no other illustrators listed. The coloring book is in great shape too, only one page colored!
